Frail seniors in Espanola are getting new assisted living services. The Victorian Order of Nurses will deliver those services thanks to funding from the North East Local Health Integration Network. Also being improved are coordination efforts between hospitals and health-care providers when dealing with hospital discharges. The LHIN says improving this area ensures seamless ongoing care for seniors once they leave a hospital setting.
